3.0 tdi 2008 shredded serpentine belt (how, why)

Heya folks so i had this unexpected failure with my serpentine belt will attach pictures, wanna know what do you think why this happened. Full set with waterpump plastic wheels ,belt + termostat and tensioner was done at 233k kilometres now at 255 and this happened see pics. That one plastic wheelie is melted below. Before this i did swirl delete mechanically and mapped too, anything i could mess up?

Perhaps checking the bearings of the components the serpentine belt is running through. As a failing bearing will be more resistant and resulting additional load to your belt hence the freying/snapping.
